Intersection of JFK Boulevard and 55th Street Bayonne, NJ On 10/05/2017 at approximately 5:30 PM the following copies of the injury and illness records were requested from the employer: OSHA 300/300A Logs (Injury and Illness Report) for 2015, 2016 and 2017. The noted records were not provided to OSHA approximately 30 business days later. Violation occurred on or about 10/05/2017
