29 CFR 1904.39(a)(1): Basic Requirement. Within eight (8) hours after the death of any employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. The employer is failing to report a fatality within eight (8) hours after the death of their employee as a result of a work-related incident. Most recently, on or about 1/26/2018, at 849 Stone Bridge Springs Drive, Chesterfield, Missouri, an employee was installing roofing materials on a roof of a residential building and an accident occurred resulting in a fatality, which was not reported to OSHA by the employer.
