29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On or about April 11, 2018 and at times prior thereto, the employer failed to report to OSHA work-related amputations and hospitalizations prior to the elapse of the 24 hour reporting period of the incidents: a) a work-related amputation occurred on March 31, 2018; and b) a work-related hospitalization occurred on April 11, 2018.
