29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks: (a) On August 25th, 2018 employees were exposed to multiple struck-by hazards while setting and securing both horizontal and vertical beams that were improperly rigged and while working underneath loads in the vicinity of 72nd Avenue NW & 38th Street NW in Parshall, ND. NOTE: Montoya Erectors, Inc, was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent, which was contained in OSHA inspection #1302434, citation #1, Item #3, and was affirmed as final order on 6/19/2018, with respect to the workplace located at Family Dollar Project, Rt. 264 Narbona Circle, Window Rock, AZ 86515
