(a) The employer is failing to provide records. At 6190 Mid Rivers Mall Drive in St. Peters, MO 63304 - On 07/24/2019 at 10:07 AM an authorized government representative requested OSHA 300 Injury and Illness Logs and 300A Summaries, which were not provided until 07/26/2019 at 07:37 AM. (b) The employer is failing to provide records. At 6190 Mid Rivers Mall Drive in St. Peters, MO 63304 - On 07/24/2019 at 10:07 AM an authorized government representative requested the Form 301, or equivalent, related to the fatality that occurred on 07/23/2019, which was not provided.
