29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness. Facility: On or about 05/13/2020, the employer was not completing 301 forms for the following workplace illnesses on the OSHA form 300 for calendar year 2020. From the time period between 03/25/2020 to 05/21/2020, forty nine (49) incidents of COVID-19 infection resulting in days away from work where 301 forms were not completed. These 49 incidents include worker illnesses that involved between 3 days away from work and 29 days away from work. .
