Location: Facility On or about 05/20/2020, a COVID-19 workplace fatality was not reported to OSHA within 8-hours as required. An employee passed away on 05/20/2020 and the employer received notification of the employee's death on 05/20/2020. As of 05/22/2020, OSHA had not been notified of the fatality.
