29 CFR 1904.39(a)(1): The employer did not report, within 8 hours, the death of an employee resulting from a work-related incident: a) Mt. Bethel Management Company, LLC, dba Mt. Bethel Village, 130 Mt. Bethel Rd., Warren, NJ: An employee, who worked as a Direct Support Staff member caring for suspected and confirmed COVID-19 residents, died from complications due to COVID-19 illness on 4/21/20. The employer did not report the employees death to OSHA until 6/25/2020. Violation occurred on or about 04/21/2020.
