29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident Facility: On or about 05/08/2020, a COVID-19 workplace fatality was not reported to OSHA within 8-hours as required. An employee passed away on 05/08/2020 and the employer received notification of the employee's death on May 10, 2020. As of August 19, 2020 , OSHA had not been notified of the fatality.
