Text For Citation: 02 Item/Group: 001 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On and/or before September 9, 2020, on Line 11 in plant 3, employees were exposed to machine hazards associated with rotating parts at or near the pasta discharge chute on the Line 11 Lyco Blancher (model: 60"X16' BLANCH; S/N: BFI 522-B11) during disassembly of the cooling drum from the blancher. The employer failed to ensure application of the energy control procedures as the Line 11 Blancher was not shut down to perform the sanitation/cleaning activity [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine caught-in hazards. On or about September 9, 2020, an employee sustained an amputation of the thumb while performing the aforementioned activity due to the Line 11 Blancher running during disassembly. b. On and/or before September 9, 2020, on Line 11 in plant 3, employees were exposed to machine hazards associated with rotating parts at or near the knockout/snowball and cup in the Line 11 Veggie Depositor 3 (model: 3800-000-022; S/N: 8713; Mfr.: FEMC Innovative Packaging Machinery) while unclogging it and while it was in operation. The employer failed to ensure application of the energy control procedures as the Line 11 Veggie Depositor 3 was not shut down to perform the unclogging activity [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine caught-in hazards. On or about September 9, 2020, an employee sustained an amputation of the middle finger while performing the aforementioned activity due to the Line 11 Veggie Depositor 3 running during the unclogging activity. Bellisio Foods, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1391535, citation number 1, item number 1a and was affirmed as a final order on June 26, 2019, with respect to a workplace located at 100 East Broadway Street, Jackson, Ohio 45640.
