On or about September 13, 2020, an employee reported a positive COVID-19 test following workplace exposures occurring September 3, 2020 and September 4, 2020. The employer did not record a work-related illness of an employee on the OSHA 300 log or its equivalent. Abatement Certification and Documentation is required
