29 CFR 1910.134(e)(6)(i)(A):The employer did not obtain from physician or other licensed health care professional (PLHCP) information on any limitations on respirator use related to the medical condition of the employee, or relating to the workplace conditions in which the respirator will be used, including whether or not the employee is medically able to use the respirator: a) Southeast VA Clinic located at 3285 S Val Vista Dr, Gilbert, AZ 85295: On or about December 28 2020, employees required to wear N95 respirators while providing care to patients had received a medical evaluation for wearing PAPRs only, and had not been evaluated for their ability to wear a N95 respirator.
