29 CFR 1910.119(d)(3)(i)(B): Process Safety Information pertaining to the piping and instrumentation diagrams (P&ID) was not compiled, complete and accurate. This violation was most recently observed on or about December 23, 2021, in Hydro Desulfurization Unit (HDU-1) at ExxonMobil refinery located in Baytown, TX, when the employer did not ensure Process Safety Information (PSI) such as Piping and Instrumentation Diagram (P&ID) # 134-500-0058, Rev 4, was complete and accurate included, but not limited to the following: a. 14-inch flanges were missing on a bypass line from furnace F-701 to reactor R-702B. b. Line number 0-084-14"-300# was missing for 14-inch bypass line. c. Normally closed (NC) symbols were missing for valves ROV-075 and ROV-605.
