29 CFR 1904.39(a)(1): Within eight (8) hours after the death of any employee as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. On or about June 21, 2022, an employee suffered injuries that resulted in a fatality. The fatality was not reported within 8 hours to the local Area Offices of the Occupational Safety and Health Administration.
