Text For Citation: 02 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees working in and around the racking system from collapse of the racking system and falling pallets and product: a) On or about August 29, 2022, employees, such as but not limited to order assemblers and porters who were picking orders and cleaning underneath pallet racks with legs that supported up to 6000 pounds and pallet support slats that supported up to 2000 pounds, were exposed to falling products that were stored in visibly damaged racks or racks that were missing pallet support slats. The employer did not ensure that the steel storage racking systems containing slots 14AF32, 14AE02, 14AD2A, 14AD3A, 05A06A, 16AS7A, 16AS63, 16AR93, 16AQ9A, and 16AR63 were not used until the repairs were performed that restored the racks to at least their previous capacity and the pallet supports/slats were replaced. b) On or about August 29, 2022, employees, such as but not limited to order assemblers and porters who were picking orders and cleaning underneath pallet racks with legs that supported up to 6000 pounds, were exposed to falling product in racks which included but were not limited to slot 15CE2A that had been installed without the anchor bolts before placing the pallet racks into service. Zenith Logistics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSHA ACT of 1970 Section (5)(a)(1) for failing to protect employees working in and around the racking system from collapse hazards, which was contained in OSHA inspection number 1446288 , citation number1, item number 1, and was affirmed as a final order on March 11, 2020, with respect to a workplace located at Zenith Logistics, Inc.
