29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: 438 Callaghan road, San Antonio, TX: On or about March 29, 2023, the employer failed to report a work-related hospitalization to OSHA within the required 24-hour period. The date of hospitalization was March 25, 2023; the employer reported the hospitalization to OSHA on March 27, 2023.
