29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided: (a) Oak Hill Baptist Church, Oak Hill, WV: Employees were wearing tennis while performing various roofing operations such as, but not limited to carrying shingles and securing shingles to the roof using Dewalt nail guns exposing employees to foot and/or toe injuries as determined on June 4, 2013. ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM MASTER ROOFING SYSTEMS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.95(a) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 309477347, CITATION NUMBER 1, ITEM 2, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 16, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT BECKLEY WV.
