29 CFR 1910.252(a)(2)(xiii)(D): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment, and did not advise all contractors about flammable materials or hazardous conditions of which they may not be aware: (a) Tank Farm - On or about July 28, 2014 employees working for Accu-Fab Construction, Inc. were allowed to perform welding and cutting on piping connected to Tank #10 that contained hydrogen sulfide and methane. The tank and piping had not been cleaned prior to the start of work.
