29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a. Throughout All Facilities - On or about September 23, 2014, the employer had not performed annual periodic inspections for the last three years on any of the roll formers, power presses, press brakes, and other operating equipment to ensure that documented lockout tag out procedures were being followed by authorized employees, thereby exposing employees to the re-energization of equipment and amputation type injuries. Edsal Manufacturing Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 313940553, citation number 2, item number 1 and was affirmed as a final order on April 20, 2011, with respect to a workplace located at 4400 S. Packers Avenue, Chicago, IL 60609.
