29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Finger jointer, machine number 3H474; On or before April 20, 2015, energy isolation devices were not located and operated in a manner to isolate energy sources while the machine operators performed servicing tasks including but not limited to blade changes and tower rotations, clearing jams and cleaning activities.
