29 CFR 1910.147(d)(4)(ii): Lockout devices utilized were not affixed in a manner that held the energy isolating device in a safe or off position. On or about 4 August 2017, and at times prior thereto, the employer failed to ensure lockout devices were affixed prior to work activities, exposing the employees to amputation hazards on an HMC Bagger during testing and positioning. John B. Sanfilippo & Son, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d)(4)(ii), which was contained in OSHA inspection number 1022069, citation number 01, item number 001/a and was affirmed as a final order on 06/03/2015, with respect to a workplace located at 1703 N. Randall Rd., Elgin, IL 60123.
