29 CFR 1904.39(a)(2): The employer did not report the in-patient hospitalization, amputation, or loss of an eye of an employee to OSHA within twenty-four (24) hours of a work-related incident. a) On or about October 19, 2018 or prior thereto: The employer had a work-related incident that led to an inpatient hospitalization of a powered industrial truck driver on September 21, 2018. The employer did not report this incident to OSHA. NOTE: Abatement certification is NOT required for this item.
