29 CFR 1904.29(a): A log of all Work-Related Injuries and Illnesses (OSHA Form 300) and for the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: On June 24, 2019, the CSHO discovered that the employer had not ensured to maintain a current OSHA 300 log and OSHA 301 incident forms for any work-related injury, illnesses or injuries that occurred in calendar year 2019.
