29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Saw Mill, Woodchip Silo; On or about May 27, 2020 and at times prior to, the employer allowed the Night Boiler Operator to conduct maintenance activities on the woodchip silo without ensuring that energy control procedures were being utilized to isolate energy sources exposing the employee to caught-in hazards. b) Saw Mill, Board Table; On or about May 28, 2020 and at times prior to, the employer allowed the Twin Saw Operator and Edger Operator to conduct maintenance such as jam clearing at the board table without ensuring that energy control procedures were being utilized by those involved in the maintenance activities exposing the employees to amputation hazards. c) Saw Mill, Scrambler; On or about May 28, 2020 and at times prior to, the employer allowed the Scrambler Operator to conduct maintenance such as belt changing without ensuring that energy control procedures were being utilized exposing the employee to amputation hazards. d) Planer Mill, Trim Saw; On or about May 28, 2020 and at times prior to, the employer allowed the Grader to conduct maintenance activities such as belt change out at the Trim Saw without ensuring that all energy control procedures were being followed in that the employee would lock out electrical sources but not pneumatic exposing the employee to amputation hazards.
