29 CFR 1910.134(d)(1)(ii):The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification: a) Nexion Health at Grenada, Inc. located at 1966 F S Hill Drive in Grenada, MS On or about 12/29/2020, the employer did not require the use of NIOSH-certified respirators by employees who were required to care for suspected and confirmed positive COVID-19 residents. The employer provided respirators labeled 3M 9501+ KN95 respirator with ear loops.
