29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness: MedStar Washington Hospital Center located at 110 Irving Street, NW Washington, DC 20010: On or about January 13, 2021, the employer had not correctly or completely entered all of the required information on an OSHA Form 301, or equivalent form, for cases including but not limited to: (a) Case #20185084 dated February 15, 2021, when an employee contracted a work-related respiratory illness resulting in 10 days away from work, and the lines for the completion date, the completed by, the title, and the phone were left blank. There were numerous additional CY2020 and CY2021 cases with this same deficiency. (b) Item 10, the case number, was left blank for a case dated April 14, 2021 when an employee suffered a work-related needlestick injury. There were numerous additional CY2020 and CY2021 cases with this same deficiency. (c) Item 17, the object or substance that directly harmed employee, was left blank for a privacy case dated April 2, 2021, that resulted in 10 days away from work, and also did not have a case number. There were numerous additional CY2020 and CY2021 cases with this same deficiency. (d) Item 12, the time employee began work, was blank on all of the CY2020 and CY2021 forms including case #20185540 dated April 13, 2021, when an employee sustained a fractured nose resulting in 8 days away from work. ABATEMENT CERTIFICATION REQUIRED FOR THIS ITEM
