29 CFR 1910.147(c)(6)(i)(B): The periodic inspection of the energy control procedure was not conducted to correct any deviations or inadequacies identified: On or about September 1, 2016, the employer did not ensure energy control procedures for the #5857 Farquar 1500 ton hydraulic press and the #2061 Blacksmith 5,000 pound steam hammer were corrected when deficiencies were identified. Deficiencies such as, but not limited to, air cooler fan motor lock out, inaccurate voltage, and hydraulic jack use as blocking between the rams and anvil bases during the changing of dies as a maintenance service task. SIFCO FORGE GROUP WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS EQUIVALENT OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.147(c)(6)(I), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1010588, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 18, 2015, AND, INSPECTION NUMBER 315443671, CITATION 2, ITEM 1 AND WAS AFFIRMED AS A FINAL ORDER ON OCTOBER 27, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 970 EAST 64TH STREET CLEVELAND, OHIO 44103.
