29 CFR 1904.29(a): The employer did not use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report. a) On or about October 19, 2018 or prior thereto: The employer had a work-related incident that led to an inpatient hospitalization of a powered industrial truck driver on September 21, 2018. The incident required medical treatment and was an OSHA recordable case. The employer has not been using OSHA forms to document recordable injury and illnesses. NOTE: Abatement certification is required for this item.
