29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) 3155 Primary Profiling Module: On or about December 21, 2022, the employer exposed employees to crushed-by hazards in that the employer failed to ensure the hydraulic energy was isolated by opening the manual hydraulic dump valve to prevent unexpected module movement when employees were engaged in servicing and maintenance activities. As a result, the applicable energy control elements, LOTO device application [(d)(4)], stored energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to safeguard employees from the machine servicing hazards.
