29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done: At the Main Facility, employee(s) engaged in manufacturing operations were exposed to laceration and amputation hazards. The employer did not provide a written certification that a workplace hazard assessment had been performed whereby the employer did not conduct any PPE hazard assessments in any areas of the facility to determine adequate PPE for employee(s). The employer is unaware if hazards exist in the facility that could be mitigated by PPE use and implementation.
