29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about June 20, 2017, at the trenching site located at the Polecat Creek Bridge on S. 49th West Avenue in Sapulpa, OK, no protective devices were in place to protect employees working in a trench that was more than 5 feet in depth. The employees were exposed to the hazard of being crushed by fall soil. Cherokee Pride, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1206000, citation number 1, item number 5b, and was affirmed as a final order on May 22, 2017, with a final abatement date of April 13, 2017, with respect to a workplace hazard at W. 81st Street and 23rd West Ave. Tulsa, OK 73132. Cherokee Pride, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1208975, citation number 1, item number 2, and was affirmed as a final order on May 22, 2017, with a final abatement date of May 13, 2017, with respect to a workplace hazard at 2500 W. 81st Street, Tulsa, OK 73132.
