29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. a) Production area 12 foot weaver machine: The employer did not report an in-patient hospitalization and an amputation that occurred on 7/30/2015 when an employee was trouble shooting the finger mechanism on the turret of a 12 foot weaver machine, on or about 7/30/2015. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
