29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: At the re-feed system, the rotary valve shaft was not guarded and projected approximately 3.75 inches from the housing exposing employees to being caught in the unguarded shaft. Johns Manville was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.219(c)(4)(i), which was contained in OSHA Inspection 1066844, citation number 1, item number 2, and was affirmed as a final order on December 15, 2015, with a final abatement date of November 24, 2015, with respect to a workplace located at 7500 Dutch Road, Waterville, OH, 43566.
