29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycle: a) On or about December 12, 2016, at the above located workplace, employees were required to work at Bartelt line number 1 (a Bartelt Form & Fill Automatic Horizontal Bagger). The line's knife blade and sealer points of operation were not guarded. The employees were thereby exposed to amputation, laceration, burn and crushing hazards. b) On or about December 13, 2016, at the above located workplace, employees were required to work at Bartelt line number 2 (a Bartelt Form & Fill Automatic Horizontal Bagger). The line's knife blade and sealer points of operation were not guarded. The employees were thereby exposed to amputation, laceration, burn and crushing hazards. The Gilster-Mary Lee Corporation was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 959973, citation number 1, item number 1 and was affirmed as a final order on August 13, 2014, with respect to a workplace located at 705 N Sparta Street, Steeleville, IL 62288.
