29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). 50 Cedar Street, Somerville, MA 02143 On or about June 12, 2017, employees were exposed to a cave-in hazard while performing work in a trench 15 to 16 feet in depth with protective system that was not installed in accordance with the design provided by a Registered Professional Engineer. P. Gioioso & Sons, Inc. was previously cited for a Willful - Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 315556936 citation number 03, item number 002 and was affirmed as a final order on 11/7/2012, with respect to a workplace located at Grant St. and C St., Framingham, MA. P.
