Northcrest Acres Nursing & Rehabilitation Center, LLC DBA Northcrest Rehab and Nursing Center located at 240 Northcrest Drive, Napoleon, OH 43545: On or about December 16, 2021, the employer failed to ensure the COVID-19 log contained one form of contact information for each employee listed on the log, occupation, location where the employee worked, the date of the employee's last day at the workplace, the date of the positive test for, or diagnosis of, COVID-19, and the date the employee first had one or more COVID-19 symptoms.
