29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. Location - 103 Tremont Street, Peabody, MA: On or about 08/16/2013, employees were not provided with and did not use eye protection while operating pneumatic nail guns. The, Daryl Provencher DBA Provencher Home Improvments was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.404 (b)(1), which was contained in OSHA inspection number 315871574, Citation number 1, Item number 2a and was affirmed as a final order on March 30, 2012, with respect to a workplace located at 26 Oakland St., Salem, MA.
