29 CFR 1910.218(f)(2)(i): Electrical power apparatus to hydraulic forging press(es) was not locked out when die(s) were being changed or maintenance was being performed on the press(es): On or about September 1, 2016, employees were exposed to a struck by caught between hazard while changing dies at the #5857 Farquar 1500 ton hydraulic press. During die changes the hydraulic energy to the ram was not shut off and locked out. SIFCO FORGE GROUP WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS SAME OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.218(f)(2)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1010588, CITATION NUMBER 1, ITEM NUMBER 8 AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 18, 2015, WITH RESPECT TO A WORKPLACE LOCATED AT 970 EAST 64TH STREET CLEVELAND, OHIO 44103.
