29 CFR 1926.1417(o)(3)(ii): The operator proceeded with a lift that exceeded 75% of the maximum rated capacity at the longest radius used during the lift operation without verifying the weight of the load in accordance with paragraph (o)(3)(i) of this section. On November 25, 2022, at Wharf 4, Gantry Crane #2: The YD-120 crane lifted an A-frame leg of the boom assembly with attached caged ladder calculated at almost two times the load capacity based on technical specification on site. Employees were exposed to struck-by and crushed-by hazards.
