29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Saw Mill, Woodchip Silo; On or about May 27, 2020 and at times prior to, the employer allowed the Night Boiler Operator to conduct maintenance activities on the woodchip silo without providing them the training to isolate energy sources exposing the employee to caught-in hazards. b) Saw Mill, Board Table; On or about May 28, 2020 and at times prior to, the employer allowed the Twin Saw Operator and Edger Operator to conduct maintenance such as jam clearing without providing them the training to isolate energy sources exposing the employees to amputation hazards. c) Saw Mill, Scrambler; On or about May 28, 2020 and at times prior to, the employer allowed the Scrambler Operator to conduct maintenance such as belt changing without providing them the training to isolate energy sources exposing the employee to amputation hazards. d) Planer Mill, Trim Saw; On or about May 28, 2020 and at times prior to, the employer allowed the Grader to conduct maintenance activities such as belt change out at the Trim Saw without providing them the training to isolate energy sources exposing the employee to amputation hazards.
