29 CFR 1910.303(b)(1): Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. a. 4400 S. Packers - Department 7 - On or about September 23, 2014, the electrical wires energizing the mechanical power press# 35 were not fully enclosed, thereby exposing employees to live electrical wires and electrocution type injuries. b. 4400 S. Packers - Department 7 - On or about September 23, 2014, the electrical wires energizing the flywheel on the riveter #4 were not fully enclosed, thereby exposing employees to live electrical wires and electrocution type injuries. c. 4400 S. Packers - Department 7 - On or about September 23, 2014, the electrical wires energizing the flywheel on the riveter #5 were not fully enclosed, thereby exposing employees to live electrical wires and electrocution type injuries. d. 4400 S. Packers - Department 7 - On or about September 23, 2014, the electrical wires energizing the flywheel on the riveter #7 were not fully enclosed, thereby exposing employees to live electrical wires and electrocution type injuries. e. 4400 S. Packers - Department 7 - On or about September 23, 2014, the electrical wires energizing the flywheel on the riveter #14 were not fully enclosed, thereby exposing employees to live electrical wires and electrocution type injuries. f. 4000 S. Racine - Department 11 - On or about September 23, 2014, the electrical wires energizing the flywheel on the riveter #30 were not fully enclosed, thereby exposing employees to live electrical wires and electrocution type injuries. g. 4400 S. Packers - Department 12 - On or about September 23, 2014, the employer failed to enclose live electrical wire at the end of conduit, thereby exposing employees to live electrical wires with a 480 voltage rating that was protruding from the conduit that was located in the middle of the aisle that employees use to walk through the department. h. 4000 S. Racine - Department 11 - On or about September 23, 2014, the employer failed to remove a riveter from service when electrical wires energizing the flywheel were causing visible sparks, thereby exposing employees to electric shock. Edsal Manufacturing Company Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(1), which was contained in OSHA inspection number 313940553, citation number 1, item number 5 and was affirmed as a final order on April 20, 2011, with respect to a workplace located at 4400 South Packers Avenue, Chicago, Illinois 60609.
