Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to electrocution from exposed live electrical parts: a)Complex 50, Barn 55 - A live 120 volt wire running out of the electrical panel on the west side was protruding through a six inch hole to the outside of the barn six foot from ground level and had nothing attached to it which exposed electrical live parts to contact. b)Complex 50, Barn 57 - The exhaust fan switch on the west wall did not have a receptacle cover which exposed 120 volt live electrical parts to contact. c)Complex 50, Barn 57 - An electrical panel control box for the manure exhaust fan on the east wall did not have a cover which exposed electrical live parts to contact. d)Complex 50, Barn 59 - The manure exhaust fan electrical supply control box on the west wall did not have a cover which exposed electrical live parts to contact. e)Complex 50, Barn 59 - The row-3-pit scraper electrical control box had no cover which exposed the barn man to 240 volt electrical live parts. f)Complex 50, Barn 59 - The electrical automatic stop control box for row 3 pit scraper did not have a cover which exposed 120 volt electrical live parts to contact. g)Complex 50, Barn 48 - Two electrical control boxes for the exhaust fans on the east wall did not have covers which exposed electrical live parts to contact. h)Complex 50, Barn 49 - The cover for the #2 manure exhaust fan was torn off which exposed electrical live parts to contact. i)Complex 50, Barn 51 - The electrical supply wires for the egg conveyor belts of cages 1,,2, 3, 4 were not secured inside their electrical receptacles which exposed 120 volt live parts to contact. j)Complex 50, Plant 5 - The electrical panel box for the #1 rod conveyor on the north wall had two circuit breaker openings which exposed electrical live parts to contact. k)Complex 80, Corridor Between Barns 81 and 82 Electrical control #2 had two circuit breaker openings which exposed electrical live parts to contact. l)Complex 60, Corridor Between Barns 64 and 66 - The electrical panel had a circuit breaker opening which exposed electrical live parts to contact. m)Complex 60, Barn 64 - Two electric light fixtures were not secured in their insulators which exposed 110 volt electrical live parts to contact. n)Complex 60, Barn 64 - A steel receptacle plug-in box was wired direct from the light fixture above it whichexposed electrical live parts and was wired in violation of National Electrical Codes. o)Complex 60, Barn 65 - The electrical control boxes for the escalator egg tape conveyor of rows 1, 2, 3, 4 on the north end had seven knockout openings in their switch control boxes which exposed 110 volt live electrical parts to contact. p)Complex 60, Barn 65 - The electrical switch control box on the west wall for the #1 exhaust fan was broken which exposed 120 volt electrical live parts to contact. q)Complex 60, Barn 65 - The electrical switch control box for exhaust fan #4 on the west wall had knockout opening which exposed 120 volt electrical live parts to contact. r)Complex 60, Barn 66 - The electrical control switch box for the manure exhaust fan on the east wall had a knockout opening which exposed 120 volt electrical live parts to contact. s)Complex 60, Barn 66 - The electrical switch control boxes on the east wall for the fourth series of ventilating fans has three knockout openings which exposed 120 volt electrical live parts to contact. t)Complex 60, Barn 67 - The pit cleaner was operating the pit scraper motor from the inside of the electrical control box which exposed him to 150 volts electrical live parts. u)Complex 60, Barn 65 - On the west wall eleven switch control receptacles for the four sets of ventilating fans did not have switch face cover plates which exposed 120 volt electrical live parts to contact. v)Snells Hill Road, Brooder Barn 4 - The electrical panel on the third floor had two circuit breaker openings which exposed electrical live parts to contact. w)Snells Hill Road, Brooder Barn 4 - The electrical switch control box for the automatic cage feeder motor in row #3 on the third floor had no receptacle cover plate which exposed 120 volt electrical live parts to contact. x)Snells Hill Road, Brooder Barn 4 - The electrical panel box for the #1 ventilating fan had live uncapped electrical wires which exposed 120 volt live electrical parts to contact. y)Snells Hill Road, Brooder Barn 4 - On the north wall the electrical supply line to the automatic feeder motor of row #3 on the second floor had two breaks in the wire which exposed 240 volt electrical live parts to contact. z)Snells Hill Road, Brooder Barn 4 - The electrical panel on the east wall of the second floor had three circuit breaker openings which exposed electrical live parts to contact. a2)Snells Hill Road, Brooder Barn 4 - The electrical panel on the east wall of the first floor had four circuitbreaker openings which exposed electrical live parts to contact. b2)Snells Hill Road, Brooder Barn 3 - The electrical control switch box for the automatic feeder motor in row #1 of the third floor had frayed and broken insulation of its wires which exposed electrical live parts to contact. c2)Snells Hill Road, Brooder Barn 3 - The electrical control switch box for the ventilating fan on the east wall of the third floor had no cover which exposed 120 volt electrical live parts to contact. d2)Snells Hill Road, Brooder Barn 3 - The electrical supply wires for the automatic feeder motor at the north end of row #1 on the second floor had no secondary insulation which exposed electrical live parts to contact. e2)Snells Hill Road, Brooder Barn 3 - The electrical panel on the west wall of the first floor had four circuit breaker openings which exposed electrical live parts to contact. f2)Pittsfield Brooder Barn - Three electrical junction boxes on the east wall of the third floor had no face cover plates with exposed electrical live parts. g2)Pittsfield Brooder Barn - The six electrical junction boxes for the automatic feeders on the third floor had no face cover plates which exposed electrical live parts to contact. h2)Pittsfield Brooder Barn - The electrical supply wires for the row #1 feeder on the east side of the third floor had no screw caps which exposed electrical live parts to contact. i2)Pittsfield Brooder Barn - The two electrical switch boxes in the stairwell of the third and fourth floors had no face cover plates which exposed 120 volt electrical live parts to contact. j2)Pittsfield Brooder Barn - On the second floor stairwell landing the six electrical switch relay panels had no panel covers which exposed 120 volt electrical live parts to contact. k2)Pittsfield Brooder Barn - The two electrical switch control boxes on the second floor stairwell landing had no face cover plates which exposed 120 volt electrical live parts to contact. l2)Pittsfield Brooder Barn - On the second floor the electrical control switch box for the middle exhaust fan bank on the east wall had no receptacle face cover plate which exposed 120 volt electrical live parts to contact. m2)Pittsfield Brooder Barn - On the second floor two electrical light receptacle boxes had no face cover plates which exposed 110 volt electrical live parts to contact. n2)Meadowbrook Brooder Barn - The electrical light junction box at the north end of the second floor hadno face cover plate which exposed 110 volt electrical live parts to contact. o2)Meadowbrook Brooder Barn - The electrical light junction box at the south end of the second floor had no face cover plate which exposed 110 volt electrical live parts to contact. p2)Meadowbrook Brooder Barn - On the first floor the electrical control panel for the exhaust fan bank on the west wall had no panel cover which exposed 120 volt electrical live parts to contact. q2)Meadowbrook Brooder Barn - The electrical panel in the first floor stairwell landing had three wire entrance knockout openings which exposed electrical live parts to contact. r2)Meadowbrook Brooder Barn - The electrical light timer box in the first floor stairwell area had no panel cover which exposed electrical live parts to contact. s2)Meadowbrook Brooder Barn - The electrical dryer receptacle outlet in the first floor stairwell area had no cover which exposed 220 volt electrical live parts to contact. t2)Montville Brooder Barn 965 - Two light fixtures on the second floor had broken insulators which exposed 110 volt electrical live parts to contact. u2)Montville Brooder Barn - The electrical plug-in receptacle in the boiler room between barns 965-966 had no receptacle cover and a broken plug-in insert which exposed 120 volt electrical live parts to contact. v2)Montville Brooder Barn - The plug-in receptacle on the back wall in the boiler room between barns 965-966 had no receptacle cover plate which exposed 120 electrical live parts to contact. w2)Montville Brooder Barn - In the boiler room between barns 965-966 the electrical junction box mounted on the steel girder had no face cover plate which exposed 120 volt electrical live parts to contact. x2)Montville Brooder Barn - The electrical panel in the boiler room between barns 965-966 had a circuit breaker opening which exposed electrical live parts to contact. y2)Montville Brooder Barn 966 - The electrical panel in the boiler room between barns 965-966 for barn 966 had three circuit breaker openings which exposed electrical live parts to contact. z2)Montville Brooder Barn - The electrical panel in the corridor of the second floor between barns 965-966 had a circuit breaker opening which exposed electrical live parts to contact. a3)Montville Brooder Barn - The electrical light timer control box on the second floor in the corridor between barns 965-966 had no panel cover which exposed electrical live parts to contact. b3)Montville Brooder Barn 966 - On the second floor corridor an electrical panel had no cover which exposed electrical live parts to contact. c3)Montville Brooder Barn 965 - The electrical panel in the corridor of the second floor had a circuit breaker opening which exposed electrical live parts to contact. d3)Montville Brooder Barn - In the generator room between barns 965-966 an electrical outlet had no receptacle cover and was not attached to the broken away cinder block wall which exposed 120 volt electrical live parts to contact. e3)Montville Brooder Barn - In the generator room between barns 967-968 two electrical receptacle outlets had no receptacle covers which exposed 120 volt electrical live parts to contact. f3)Montville Brooder Barn - The electrical light timer panel box in the electrical panel room between barns 967-968 had no panel cover which exposed electrical live parts to contact. g3)Montville Brooder Barn - An electrical panel in the panel room between barns 967-968 had two circuit breaker openings which exposed electrical live parts to contact. h3)Montville Brooder Barn - In the boiler room between barns 967-968 an electrical panel had one circuit breaker opening which exposed electrical live parts to contact. i3)Montville Brooder Barn - An electrical receptacle in the boiler room between barns 967-968 had no receptacle cover which exposed 120 volt electrical live parts to contact. j3)Montville Brooder Barn 967 - At the entry way of the barn on the second floor an electrical junction box had no face cover plate which exposed 110 volt electrical live parts to contact. k3)Monroe Brooder Barn - In the boiler room between barns 944-945 the Taco Cartridge Circulator control had no face cover plate which exposed 110 volt electrical live parts to contact. l3)Monroe Brooder Barn - In the boiler room between barns 944-945 the electrical relay switch control panel had no cover door which exposed 110 volt electrical live parts to contact. m3)Monroe Brooder Barn, Boiler Room Between Barns 944-945 - The electrical switch control for the circulator had no face cover plate which exposed electrical live parts to contact. n3)Monroe Brooder Barn - In the boiler room between barns 944-945 the electrical panel identified with the marking(P-2-B) had four circuit breaker openings which exposed electrical live parts to contact. o3)Egg Processing Plant No. 1 - There were two breaker openings in the electrical panel near the USDA office with exposed live parts exposed to contact. p3)Complex 70, Barn 76 - The manure auger control switch did not have a cover which exposed live electrical parts.q3)Complex 10, Barn 11 - The reset control panel for the pit scrapers did not have a cover which exposed live electrical parts. r3)Complex 70, Barn 79 - Three electrical control boxes for the dropping board scrapers were defective which required the employees to open the doors and place their hands in the boxes to reverse the contacts which exposed them to 240 volts. s3)Complex 70, Barn 75 - Three electrical control boxes for the dropping board scrapers were defective which required the employees to open the doors and place their hands in the boxes to reverse the contacts which exposed them to 240 volts. t3)Complex 70, Barn 76 - Four electrical control boxes for the dropping board scrapers were defective which required the employee to open the doors and place their hands in the boxes to reverse the contacts which exposed them to 240 volts. u3)Complex 70, Barn 78 - Four electrical control boxes for the dropping board scrapers were defective which required the employees to open the doors and place their hands in the boxes to reverse the contacts which exposed them to 240 volts. v3)Complex 70, Barn 70 - The motor control switch for the manure auger did not have a cover which exposed live 120 volt electrical parts. w3)Complex 30, Barn 32 - At the far end of the barn there were two light fixtures that were broken with exposed 110 volt live electrical parts. x3)Egg Processing Plant No. 3, Small Cooler Warehouse There was an opening in the front of the lighting panel with exposed live 110 volt electrical parts. y3)Complex 10, Barn 2 - Two electrical control boxes for the pit scrapers did not have covers which exposed the barn man to 240 volt live electrical parts. z3)Complex 50, Barn 58 - On the west wall of the barn near the manure auger there was an electrical panel that was damaged and did not have a cover which exposed the barn man to 220 volt live electrical parts. a4)Complex 20, Barn 20 - At the first and second fan banks there were openings in the electrical panels for the fan controls. b4)Plant 8, #6 Egg Carton Machine - Employees performing maintenance on the electrical control box were exposed to 120 volt terminal connectors. c4)Barn #79 (1/25/96) - An electrical junction box for an exhaust fan had exposed live wires. Junction box and surrounding electrical equipment were noted to be covered with dust. d4)Barn #77 (1/25/96) - A damaged light fixture had exposed live wires. e4)Barn #77 (1/25/96) - A damaged light fixture had exposed live wires. Electrical light fixture was noted to be covered with dust. f4)Barn #77 (1/25/96) - A damaged light fixture had exposed live wires. Electrical light fixture was noted to be covered with dust. g4)Barn #76 (1/25/96) - Electric motor for an exhaust fan had exposed live wires. Motor and fan were noted to be covered with dust. h4)Trailer #34, North Bedroom - The two-bulb light fixture, which was mounted on the ceiling and missing a light bulb, was not covered or protected with an approved globe or enclosure. The fixture was physically damaged and a 120 volt tab was exposed. i4)Trailer #36, Kitchen - The Kelvinator refrigerator was not grounded, and the metal frame of the refrigerator contained 52 volts. j4)Trailer #8, West Bedroom - The two-bulb light fixture, which was mounted on the ceiling and missing a light bulb, was not covered or protected with an approved globe or enclosure. The plastic frame of the fixture was physically damaged, and the 120 volt tab was exposed. k4)Trailer #26A, East Hall - The metal frame of the onebulb light fixture, mounted on the ceiling, was not grounded. The metal frame contained 54 volts. l4)Trailer #26A, East Bedroom - The two-bulb light fixture, mounted on the ceiling and missing a bulb, was not covered or protected with an approved globe or enclosure. The metal frame of the fixture contained 64 volts. m4)Trailer #26A, East Bedroom - The face-plate cover screw on the electrical light switch was not grounded and contained 52 volts. n4)Trailer #26A, Bathroom - The metal frame on the exhaust fan was not grounded and contained 77 volts. o4)Trailer #28, Kitchen - The G.E. refrigerator was not grounded. The outside metal frame of the refrigerator contained 59 volts. p4)Trailer #31, Master Bedroom - The light switch, which was missing a cover or face-plate, contained reverse polarity. The outside metal frame of the switch contained 120 volts. q4)Trailer #31, Full Bathroom - The metal frame on the exhaust fan was not grounded and contained 51 volts. r4)Trailer #38, Kitchen - The Kelvinator refrigerator was not grounded, and the metal frame of the refrigerator contained 57 volts. s4)Hatchery, Washroom - The switchbox for the washer motor was not provided with a cover, exposing live electrical parts to employee contact. Among other methods, one feasible and useful method of abatement would be to provide covers and enclosures on exposed live electrical parts.
