29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap: On or about July 7, 2020, and at times prior thereto, the employer failed to ensure that a 110/120V extension cord that was spliced and had internal conductors damaged was taken out of service. The extension cord was plugged into the 120 V Tack Life HGP70, 1500 W heat gun. Employees exposed to an electrical shock hazard.
