29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 8/18/2015, Throughout the facility: Employees were not utilizing lock out procedures while performing work such as changing dies on draw machines, draw bench and performing cleaning and maintenance on the extruder and draw bench. b) On or about 11/5/2015, Draw bench and Extruder: Specific lock out procedures were not developed and utilized for the Draw Bench, Draw Bench Saw and the Extruder. This equipment has energy sources such as, but not limited to electrical, heat, and pneumatic. Abatement documentation is required for this item. Tecnofil Chenango, SAC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i), which was contained in OSHA inspection number 881463, citation number 1, item 9a and was affirmed as a final order on 7/3/2013, with respect to a workplace located at 40 South Main Street, Sherburne, NY.
