29 CFR 1910.147(d)(4)(i): Lockout or tagout devices are not affixed to each energy isolating device by authorized employees: (a) Machine number WR196, Heian 8 head CNC router; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to router bit changes and job setup; (b) Machine number WR209, Heian ZR-842P CNC router; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to hydro head changes; and (c) Machine number WR220, Heian ZR-842P CNC router; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to hydro head changes. Ashley Furniture Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection # 313176380, Citation 1, Item 1 issued on March 2, 2010 and was affirmed as a final order on February 7, 2011, with respect to a workplace located at One Ashley Way, Arcadia, WI.
