29 CFR 1910.147(d)(4)(i): Lockout or tagout devices are not affixed to each energy isolating device by authorized employees: (a) Machine number FH006, foam cutter/slitter; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to blade changes; and (b) Machine number FV006, foam cutter/slitter; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to blade changes. Ashley Furniture Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection # 313176380, Citation 1, Item 1 issued on March 2, 2010 and was affirmed as a final order on February 7, 2011, with respect to a workplace located at One Ashley Way, Arcadia, WI.
