29 CFR 1910.147(d)(4)(i): Lockout or tagout devices are not affixed to each energy isolating device by authorized employees: (a) Machine number WS007, Dakota panel saw; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to entering the enclosure to clean under and inside the saw housing; (b) Machine number WS121, Schelling panel saw; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to blade changes; (c) Machine number WS127, Schelling panel saw; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to blade changes and cleaning under and around the saw; (d) Machine number WS129, Schelling panel saw; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to blade changes and cleaning under and around the saw; (e) Machine number WS134, Schelling panel saw; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to blade changes; and (f) Machine number WS137, Schelling panel saw; lockout or tagout devices are not applied to isolate all energy sources while machine operators perform servicing tasks including but not limited to blade changes.
