29 CFR 1904.29(a): A Log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: (a) Office Area: The OSHA Form 300 or equivalent was not used to record occupational injuries and illnesses that occur in the workplace. A recordable injury, resulting in two missed days of work, occurred February 19, 2014, and was not recorded on an OSHA 300 log, as observed on or about March 5, 2014.
