29 CFR 1910.212(a)(3)(ii): The point of operation machine guards were not designed or constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) Polar Service Center, 4850 E 74th Ave., Commerce City, CO 80022: On or before July 24, 2014, the employer did not ensure that guarding on a Pacific hydraulic press was adequate. The wire used for guarding was not taunt and allowed the operator to reach the point of operation. Abatement Note: Abatement certification is not required for this item.
