Investigation Summary
At approximately 1:00 p.m. on January 5, 2017, an employee was installing a test ball in the plumbing for testing the line for leaks. The side wall of the trench caved in, crushing the employee. The employee died of asphyxia when the dirt covered him up to his abdomen.
